This CD provides synopses and accompanying graphics of the 10 articles in the November 2004 Supplement to the American Journal of Kidney Diseases (AJKD) entitled: “Evidence for Improving Patient Care and Outcomes.” The supplement and this CD were developed through a collaborative effort by the National Kidney Foundation (NKF) and the University Renal Research and Education Association (URREA).
